You must be a member in good standing prior to the start of school to receive the discounted synagogue rate.
If the same as parent 1, you may leave blank.
Half-Day is not applicable for Infants.
3 Days not applicable for Infants or Pre-K Students.
My child is up-to-date on their immunization requirements for their age.
Please note that our school does not accept religious exemptions. Please contact us for further questions.
My child may have his/her picture appear in Day School advertising/promotion needs and UNTAGGED on the school Facebook page.
The registration fee is per family, not per child. This fee is non-refundable.
